Purpose of Collection
Quick Clinic Connect collects your personal information for the purpose of managing a Family Doctor Waiting List and helping connect you with a family physician when availability arises. Joining the waiting list does not guarantee placement with a family doctor and does not establish a physician-patient relationship at this time.
Custodian and Authority
Quick Clinic Connect acts as a custodian of your information under applicable Canadian privacy legislation. Your information is collected, used, and disclosed for administrative and care-coordination purposes only.
Information Collected
We collect only the minimum information necessary, which may include:
First and last name
Email address
Phone number
City and preferred area
Preferred clinic location
Physician gender preference, if any
Use and Disclosure of Information
Your information will be used to manage the Family Doctor Waiting List and to facilitate potential attachment to a family physician.
Clinics Currently Accepting Patients (Required Consent)
Your contact information may be shared with family physician clinics that are currently accepting new patients for the purpose of offering you an opportunity to register with a family doctor. This disclosure is essential to the operation of the waiting list.
New or Future Clinics (Optional Consent)
New family physician clinics may open or begin accepting patients in the future. With your permission, your contact information may also be shared with such clinics to help connect you with a family doctor. You may decline this option without affecting your place on the waiting list.
Safeguards and Storage
Quick Clinic Connect uses reasonable administrative, technical, and physical safeguards to protect your information, as required by the HIA:
Access is limited to authorized staff
Information is stored securely
Information is retained only as long as necessary for waiting list purposes or as required by law
No Medical Advice or Care
This waiting list is an administrative service only and does not provide medical advice, diagnosis, or treatment. If you require medical care, please contact a healthcare provider or seek urgent care as appropriate.

Withdrawal of optional consent does not affect your eligibility for the waiting list.
